What is Breast Augmentation and how does it work?

Breast augmentation, or augmentation mammoplasty, is done to increase breast size. The surgery can be performed using different types of breast implants (silicone – filled, saline – filled or textured) or fat transfer. During the surgery, incisions are made in specific areas where the visibility of scars is minimal – the possible locations are in the lower fold of the breast, around the areola, in the armpit or under the breast fold. The incision creates an opening to insert a breast implant, which is typically placed either on top of the pectoral muscle (also known as subglandular placement) or under it (otherwise called submuscular or subpectoral).

Breast Augmentation with implants vs. Breast augmentation with your own fat tissue

Both breast implants and fat grafting have their pros and cons. Having breast enlargement with your own fat tissue will feel more natural and leave almost invisible scarring. In addition, there is virtually no maintenance needed when compared to breast implants. Moreover, the risk of having an allergic reaction to the transferred tissue is nonexistent, as your own tissue is used. However, using fat grafting may not satisfy patients who seek a more dramatic change in size, and there is a chance of calcification at the fat injection sites.

What type of surgeries Breast implants could be combined with?

There are various procedures that are done in combination with breast augmentation:

  • Breast augmentation and mastopexy (breast lift). Combining these two techniques can lead to the best possible cosmetic results, as it diminishes the need for a larger implant and achieves a more desirable contour of the breasts.
  • Breast augmentation and tummy tuck. This is a suitable choice for those who need a lift in the abdominal  area in addition to breast enlargement.
  • Breast augmentation and liposuction. In addition to getting augmentation mammoplasty, areas with unwanted fat may be removed via liposuction. Combining these surgeries will result in an overall smoother body contour.

Is Breast augmentation with implants safe? Pros and cons of Breast implants.
Breast implants are deemed safe, as no associations between breast implants and health disorders have been found in scientific research. However, just as with any type of surgery, one must be aware of possible risks which include:

Bleeding;
Allergic reactions;
Infection;
Fluid accumulation (seroma);
Persistent pain;
Changes in sensation (nipple or breast);
Capsular contracture (tight scar tissue forming around the implant);
Implant leak or rupture;
Faulty position of the implant.
Breastfeed with implants
It is important to note that even though one may still be able to breastfeed after having breast augmentation, the procedure can possibly reduce the production of breast milk. The degree of reduction depends on the location of both the implant and the incision, as well as sensitivity of the nipple. One should also be aware that breast implants are not made to last a lifetime – factors such as age and changes in weight may affect the appearance of your breasts. Moreover, breast implants will affect future mammograms.
Who is a proper candidate for Breast augmentation with implants? When does a patient need a Breast Lift as well as Breast Augmentation?
The most common reason for getting breast implants is to improve one’s appearance if one thinks their breasts are too small. In addition, you may consider getting augmentation mammoplasty if your breasts have gotten smaller after pregnancy or are visibly asymmetrical because of development issues or past surgeries. Before the surgery the surgeon will examine your overall health and ask you about any type of medications you take or have taken in the past
If you smoke, you will be asked to quit smoking for at least 4 weeks before the surgery.
Furthermore, you will be asked to refrain from taking anti – inflammatory drugs (including aspirin) before breast augmentation surgery, as taking such medications may increase the risk of bleeding.
A baseline mammogram might be suggested so that changes in your breast tissue are recognised in the future.
You should discuss your expectations and any type of questions you have about the surgery with your surgeon during consultation. In addition, your surgeon will describe the possible augmentation mammoplasty techniques and types of implants.

I discussed with her all the things I had wanted to change about my body for a number of years. For so many of us finding the right Dr in plastic surgery is always a daunting task, particularly as it’s a cosmetic desire to look how you imagine in your mind, for me it was based on finding an old photograph of many years ago! My concerns started with feeling comfortable to discuss what I want to achieve, comfortable and confident about the ability of the Dr I may choose if I decide to proceed, the realistic care recovery process in another country if I considered going outside the UK where I had heard prices are cheaper and knowing how expensive all these “wants” would be in the UK.

Dr Ignatieva made me feel at ease and invited me to visit her in Kiev Ukraine and to only have a consultation with her and her colleague, and suggested not to make any forward planned commitments before going away to think about what I wanted to do after the consultation. Knowing she had worked on some complicated/complex/major procedure’s it gave me the confidence to at least go to see her. I went to Kiev in February 2019 and made the decision to have a bust reduction/reconstruction done in May 2019, this was major surgery given I wanted to go from a GG to a C/D cup in bra size, from start to finish the whole process made me relaxed, I was monitored frequently to make sure everything was okay and 2 weeks later on my way back to the UK a very happy patient, within 4 months of my surgery I was back to doing all the fitness training I had been comfortable to do before I felt like “my breasts were getting in my way”.



I had set out at the beginning that I wanted 3 different procedures done, it was suggested to focus only on the major surgery in the beginning, my breast reduction/reconstruction, if I was happy with the result then was the right time to discuss the rest of my “wish list”, which was liposuction to removed fat from my back and upper arms plus skin removal from my upper arms to give a nice tight skin appearance once the fate was removed. Not only have I done extensive fitness training over previous years and cut back drastically during the past few years which took a toll on the appearance of my body with muscle turning to fat, I spend a lot of time in the sun, which is not good for the skin appearance other than getting a tan and at 61 years old my ageing skin was all going against me, hence the dreadful “bingo wings” had to be reduced to a more appealing look. I have had both of these procedures done in the past month, while I’m still coming out of the end of the FULL RECOVERY stage I already know I’m 100% happy with the end result, with little restriction within days of the surgery because it was not so invasive, but to treat my body with respect, all made possible by Dr Ignatieva who made it all stress-less by the care, consideration she showed me but more important her desire to “make my body back to beautiful”. I have achieved all what I wanted to achieve without spending a huge amount of money that the UK would have cost, despite the cost the procedures were carried out in wonderful surroundings with up to date modern technology.

How can I make the payment?

Does it have to be general anaesthesia?
Local anaesthesia is applied when the surgery is minor and localized on specific area, for example otoplasty surgery.
Can I combine two surgeries at once?
In most cases combining two procedures is possible. However, plastic surgery is very individual matter, thus each case is different. The surgeon decides whether two or more surgeries may be combined after evaluation of a patient’s health condition, surgery technique, surgery duration, etc.
Are there any age restrictions for cosmetic surgery?
Unlike reconstructive surgery, cosmetic surgery is not recommended for patients under 18 years old. Patients should understand that puberty is a period of time when the most significant change of body contour occurs and there is no need to rush in surgical correction of any part of it. Moreover, in most cases, the part of body teenagers are unsatisfied with matures does not require any interference of a surgeon.
As for older patients, there is no upper age limit.
What should I bring with me?
Patients, who take medication, that was approved by our anaesthesiologist, should bring the medication with them, so it can be taken as prescribed. Moreover, you may want to bring a computer, a book, or a magazine, to keep you entertained while in the clinic or the hotel.
What questions should I ask the surgeon?
The goal of consultation is to arrange surgery details, express your expectations on surgery results and clear up your mind. Do not be afraid to ask questions concerning your decision even if you think it is not important.
Moreover, ask surgeon about his experience in particular cosmetic surgery. You will feel more comfortable knowing that person who is likely to operate on you is well known in this field.
One more helpful tip is to ask to show pictures of his previous work. You will see the result plastic surgery provides and get a chance to evaluate whether you are happy with it.

Is it safe to fly after cosmetic surgery?
Flying immediately after surgery carries a higher risk of blood clot formation and there is no guarantee that it will not be fatal. In order to prevent it, we advise all our patients to stay in clinic for several days. Surgeon will recommend necessary precautions – medications preventing from blood clot formation, moving around the ward or wearing compression stockings. Only after careful evaluation of your health state, you will be allowed to travel home.
Can I get a sick note from the surgeon? If not, how can I get it?
The surgeon that will be performing your surgery may write an official recommendation about the post-surgery period to your GP. Your GP then will provide you with an official sick note.
What is recovery period after surgery like?
You should be prepared that the final result will not be visible until several months after surgery. Most likely, the surgical area will be painful and swollen. It is normal reaction to tissue damage caused by surgical manipulation. In order to reduce uncomfortable feelings medication will be prescribed, however it does not mean that the pain will be unbearable.
Every patient is individual and copes with pain differently. Your stay in hospital should last about 1-3 days. It depends on the type of surgery performed and the patient operated. During the stay, you will be encouraged to walk in the ward. Day by day you will be allowed to return to your daily activities.
When should I expect to see the final result?
The final result is usually visible 6 months after the surgery. However, the full recovery time depends on the surgery and in some cases may take up to 1 year. The final result is visible after the bruising is gone and both inner and outer scarring process is over. Please note, that scars may fade (become less visible) for 2 years after the surgery.
What about scarring after cosmetic surgery?
As with any surgery where incision or skin cut is present, a scar will be the result of natural healing process. However, in cosmetic surgery incisions run along natural skin folds or by the hairline decreasing the possibility of visible scar. Also special medical ointments will be prescribed. Most likely you will have to apply it several times a day for a month or two. It is important to follow instructions precisely to achieve the most pleasing result.
Will I receive supporting garment or should I bring one with me?
You will receive one set of supporting garments free of charge. You may also purchase the second set in the clinic in case you want to have a spare.
How many check-ups will I have after the surgery?
Usually, Dr. Ignatyeva conducts 3-4 examinations with the patient - at 1, 3, 6 months, and 1 year after the operation +-5 calendar days. Additional visits upon the patient's request are charged according to the clinic's price list. Examination of the patient outside the doctor's working hours is possible with double tariff.

An overnight stay is needed after breast augmentation surgery.
Both soreness and swelling are expected for at least a few weeks after the surgery, as well as sensitivity to touch or sudden movements.
Your surgeon may prescribe pain medication and antibiotics for infection prevention.
Furthermore, you will need to wear a compression garment or a surgical bra.
You should follow the instructions of your surgeon regarding physical activities – it is usually recommended to avoid any type of strenuous activities for two weeks, as it is possible to cause bleeding to areas around the breast implant.
It is important to mention that one should avoid sleeping on the stomach as well as raising their arms above their head, as this can elevate the risk of bleeding, and subsequently, the formation of a haematoma.
As with any surgery, scarring is expected and is simply a part of the healing process. As mentioned, the augmentation mammoplasty incisions are done in areas where it is easy to diminish the visibility of scars. Even though the scarring process is individual for each person, there are measures to minimize the appearance of scars – you can ask your surgeon for advice on how to treat your skin before and after the surgery.
